Revival Medicine

Privacy Policy

Last updated: October 19, 2026. This policy replaces our previous Privacy Policy, effective July 30, 2024.

Your privacy is our priority. This policy covers our website, forms, emails, text messages, and events. It explains what information we collect when you visit revivalmedicine.com, sign up for updates, register for an event, or contact us.

Once you become a patient, the health information you share with us is protected health information under HIPAA. It is governed by the privacy notice you receive and sign through our secure patient portal, not by this policy. Please do not send medical details through website forms, general email, or text message. Use the secure patient portal instead.

"You" and "your" refer to anyone who visits our website or interacts with us online.

By using our website or submitting your information to us, you agree to the practices described in this policy.

2. Information We Collect

Information You Give Us

We do not collect personal information about you unless you choose to provide it, for example when you fill out a form, register for an event, join our email list, send us a message, or make a payment.

  • Contact details, such as your name, email address, and mobile phone number.
  • Form responses, such as the services you are interested in, how you heard about us, and questions you send us.
  • Event registrations, including sign-ups made by scanning a QR code at an event.
  • Communication preferences and consent records, including whether and when you agreed to receive emails or text messages.

Information Collected Automatically

  • Device and usage data, such as your IP address, browser type, device type, domain name, access times, pages visited, and the website that referred you to us. We use this to operate the website, maintain its quality, and understand general usage.
  • Cookies and similar technologies that help our website function, remember your preferences, and show us how visitors use the site. See Section 8.

Payment Information

Payments are handled by secure third-party payment processors. We do not store full card numbers on our website.

4. Text Messages (SMS)

If you provide your mobile number and check the SMS consent box on one of our forms, you agree to receive text messages from Revival Medicine. Consent is never a condition of purchase or of receiving care.

  • Message frequency varies. Message and data rates may apply.
  • Reply STOP at any time to stop receiving messages. Reply HELP for help.
  • We never include clinical details in text messages.

We do not sell, rent, or share your mobile phone number or SMS consent with any third parties or affiliates for their marketing or promotional purposes. Text messaging originator opt-in data and consent will not be shared with any third parties, excluding aggregators and providers of the text message services used to deliver our messages.

Full SMS program terms are in our Terms and Conditions.

5. Email

If you give us your email address, we may contact you about your inquiry, appointments, and services. If you join our email list, we may also send newsletters, educational content, event invitations, surveys, and offers. Every marketing email includes an unsubscribe link. Unsubscribing from marketing emails does not stop appointment or account messages that are necessary to provide our services. It may take a few business days to fully process your request.

6. How We Share Your Information

We do not sell, rent, or lease your personal information or our contact lists to anyone. We share information only in these situations:

  • Service providers who help us run our practice, such as our secure CRM and marketing platform, website hosting, email and text message delivery, our patient portal and electronic health record system, and payment processing. These providers may use your information only to perform services for us and are required to keep it confidential.
  • Legal requirements, when we believe in good faith that disclosure is required by law or legal process, or is necessary to protect the rights or property of our practice, or to protect the personal safety of our patients or the public in an emergency.
  • Business transfers, if our practice is involved in a merger, sale, or transfer of assets, subject to applicable law.
  • With your permission, for any other purpose you agree to.

We may share aggregated or de-identified information that cannot reasonably be used to identify you.

7. Your Choices and Rights

  • Access and correction. You may ask what personal information we hold about you and ask us to correct it.
  • Deletion. You may ask us to delete your personal information and to direct our service providers to delete it.
  • Marketing opt-out. You may unsubscribe from emails or reply STOP to text messages at any time.
  • Nevada residents. Nevada law gives you the right to request that we not sell certain personal information. We do not sell personal information, but you may still submit a request using the contact details below.

We may not be able to delete information we need to provide care or a service you requested, complete a transaction, detect and prevent fraud or security incidents, keep medical and financial records we are legally required to retain, or comply with another legal obligation. If we cannot fully honor a request, we will tell you why.

To make a request, contact us using the information in Section 14. We may need to verify your identity before acting on it. Requests about your medical records are handled through our secure patient portal, under the privacy notice you signed as a patient.

8. Cookies and Tracking

If you do, some parts of our website may not work as intended. Our website does not currently respond to "Do Not Track" browser signals.

9. How We Protect Your Information

We use reasonable administrative, technical, and physical safeguards to protect your information, including encrypted (SSL/TLS) connections when information is sent through our website, and we work with platforms designed for healthcare privacy. No method of transmission over the internet, a wireless network, or electronic storage is completely secure, so we cannot guarantee absolute security. If we are required by law to notify you of unauthorized access to your information, we will do so as the law requires.

10. How Long We Keep Information

We keep personal information for as long as needed for the purposes described in this policy, or as long as required by law. Medical records are retained according to Nevada medical record retention requirements.
