Revival Medicine

Terms and Conditions

Last updated: October 19, 2026

If you are having a medical emergency, call 911 or go to the nearest emergency room. Do not use our website, email, text messages, or patient portal for urgent or emergency needs.

1. About These Terms

These Terms and Conditions ("Terms") apply to your use of revivalmedicine.com, our online forms, emails, text messages, and events, and to your purchase of our services. Revival Medicine is the business name of Gregory Health PLLC, a Nevada medical practice. "Revival Medicine," "we," "us," and "our" refer to Gregory Health PLLC. "You" and "your" refer to website visitors, prospective patients, and patients.

By using our website or purchasing our services, you agree to these Terms and to our Privacy Policy. If you do not agree, please do not use our website or services.

When you become a patient, you will also complete intake forms and consents through our secure patient portal. Those documents govern your clinical care. If anything in them conflicts with these Terms, the signed patient documents control.

2. Website Content Is Not Medical Advice

Information on our website, social media, emails, events, and other educational content is for general information only. It is not medical advice, diagnosis, or treatment, and it is not a substitute for an individual evaluation by a qualified healthcare provider.

Visiting our website, filling out a form, attending an event, or contacting us does not create a provider and patient relationship. That relationship begins only after you complete our intake process and your consultation with one of our licensed providers.

3. Who Can Use Our Services

  • You must be at least 18 years old.
  • Our providers are licensed in Nevada. For telehealth visits, you must be physically located in Nevada at the time of each visit.
  • You agree to provide accurate and complete information about your identity, health history, and medications, and to keep it up to date.

4. Telehealth

We offer in-person visits in Reno and telehealth visits for patients located in Nevada. Telehealth has limits, including the absence of a hands-on physical exam and the possibility of technical problems. Your provider may decide that an in-person visit, lab work, imaging, or referral is needed before recommending or continuing treatment. You will be asked to review and sign a telehealth consent before your first telehealth visit.

5. Appointments, Cancellations, and No-Shows

Consultations and visits are booked through our secure patient portal. We reserve extended time for each appointment, so please give us as much notice as possible if you need to reschedule.

We do not charge a cancellation or no-show fee. If you miss or cancel a prepaid consultation, your payment moves to your rescheduled visit.

6. Fees and Payment

  • Cash-pay practice. Revival Medicine does not participate in insurance networks and does not bill insurance for our services. Some outside services, such as lab work at a partner lab, imaging, or pharmacy costs, may be billed separately and may be eligible for insurance through those providers.
  • Payment is due at the time of purchase or service unless you have enrolled in a payment plan. Payments are processed by a secure third-party payment processor.
  • What is included. Each consultation, program, and membership includes only the items described for that service at the time of purchase. Medications, laboratory testing, imaging, outside procedures, specialty testing, supplements, and referrals are billed separately unless specifically stated as included.
  • Price changes. We may change our prices at any time. Changes do not affect a service you have already purchased.

Good Faith Estimate

Under the federal No Surprises Act, if you are not using insurance, you have the right to receive a Good Faith Estimate explaining how much your medical care will cost. You can ask us for a Good Faith Estimate before you schedule a service. If you receive a bill that is at least $400 more than your Good Faith Estimate, you can dispute the bill. For questions or more information about your right to a Good Faith Estimate, visit www.cms.gov/nosurprises.

7. Programs and Payment Plans

Our programs run for a set length of time and may be paid in full or through a monthly payment plan where offered. By choosing a payment plan, you authorize us to charge your payment method on the agreed schedule until the program balance is paid. A payment plan is a way to pay for a program over time. It is not a month-to-month subscription, and you remain responsible for the full program price if you stop participating early, except as stated in Section 9.

If a scheduled payment fails, we will contact you to update your payment method. Services may be paused until your account is current.

8. Memberships

Continuing-care memberships renew and bill automatically each month on the same date until you cancel. By joining a membership, you authorize us to charge your payment method monthly.

  • Cancelling. You may cancel at any time by contacting us through your secure patient portal. Please cancel at least one month before your next billing date. If your cancellation reaches us less than one month before that date, your next monthly payment will still be processed, and your membership will end at the end of that billing month.
  • After cancelling. Your membership stays active through the end of the month you have paid for. We do not refund any part of a month that has already been paid.
  • Memberships are not insurance and do not replace health insurance or emergency care.
  • We may end a membership if payment fails and is not resolved, or if continuing care is no longer clinically appropriate. Prescriptions and ongoing treatment will be handled responsibly as required by law.

9. Refunds

Care that has already taken place is not refundable. This includes a consultation once it has happened, laboratory testing once your blood has been drawn, any treatment or procedure already performed, and any membership month that has already begun.

Programs are not refundable once purchased, including any part of a program you do not use. Specific exceptions may apply. For example, our providers are licensed only in Nevada, so if you move out of Nevada we may no longer be able to continue your care. If something like this comes up, please contact us and we will look at your situation individually.

For a missed or cancelled consultation, see Section 5.

Approved refunds are returned to the original payment method. Please contact us before disputing a charge with your bank or card issuer so we can try to resolve the issue directly.

10. Treatment, Medications, and Results

  • Clinical judgment. All treatment decisions, including whether to prescribe a medication, which medication, and at what dose, are made by your provider based on your evaluation. Purchasing a consultation, program, or membership does not guarantee that any particular medication or treatment will be prescribed.
  • Pharmacies. Prescriptions are filled by licensed pharmacies. Some medications and supplies may be shipped directly to your home.
  • Your part. Results depend on many factors, including your health history, how your body responds, and following your treatment plan. We cannot guarantee specific results, such as a certain amount of weight loss or relief of a particular symptom.
  • Tell us about changes. Let your provider know about side effects, new medications, or changes in your health. Do not start, stop, or change a prescribed medication without talking with your provider.

11. Patient Portal and Communication

Our secure patient portal is the best way to reach your care team about symptoms, side effects, and treatment questions. Messages are answered during business hours, and response times may vary. The portal, email, and text messages are not monitored for emergencies.

12. SMS Messaging Terms

These terms apply to the Revival Medicine text messaging program.

Program Description

If you provide your mobile number and check the SMS consent box on one of our forms, you agree to receive text messages from Revival Medicine. Messages may include appointment confirmations and reminders, rescheduling notices, event information, responses to your questions, account and billing notices, and, if you have agreed to receive them, educational content and offers. We never include clinical details in text messages.

Consent

SMS consent is collected through a checkbox that is never pre-checked. By opting in, you confirm that you are the account holder or authorized user of the mobile number provided. Consent is not a condition of purchase or of receiving care. We keep time-stamped records of opt-ins.

Frequency and Cost

Message frequency varies based on your appointments, services, and the updates you have signed up for. Message and data rates may apply. Contact your wireless provider for details about your plan.

How to Opt Out

Reply STOP to any message at any time. You will receive one message confirming you have been unsubscribed, and you will not receive further text messages unless you opt in again.

Help

Reply HELP for help, or contact us at [email protected] or (775) 234-8694.

Carriers

Carriers are not liable for delayed or undelivered messages. Our program is supported by major U.S. wireless carriers, and carrier support may change without notice.

Privacy

We do not sell, rent, or share your mobile number or SMS consent with third parties or affiliates for their marketing purposes. See our Privacy Policy for details.

13. Email Communications

If you give us your email address, you agree that we may contact you by email about your inquiry, appointments, and services. If you join our email list, we may also send newsletters, educational content, and offers. You can unsubscribe from marketing emails at any time using the link in any email.

14. Testimonials and Reviews

Testimonials and reviews on our website and social media reflect the experiences of individual patients. Individual results vary, and a testimonial is not a promise that you will have the same experience or outcome. We welcome honest reviews of our practice.

15. Use of Our Website

The content on our website, including text, graphics, logos, and program descriptions, belongs to Gregory Health PLLC or its licensors and is protected by copyright and trademark law. You may view and print content for your own personal, non-commercial use. You may not copy, republish, or sell our content without our written permission.

You agree not to misuse our website, including by attempting to gain unauthorized access, introducing harmful code, collecting other users' information, submitting false information, or using the website for any unlawful purpose.

16. Third-Party Websites and Services

Our website may link to or rely on services we do not control, such as our patient portal, payment processor, lab partners, pharmacies, and social media platforms. Your use of those services is subject to their own terms and privacy policies, and we are not responsible for their content or practices.

17. Disclaimers and Limitation of Liability

Our website and its content are provided "as is" and "as available," without warranties of any kind, to the fullest extent permitted by law. We do not warrant that the website will be uninterrupted, error-free, or free of harmful components.

To the fullest extent permitted by law, Gregory Health PLLC and its owners, providers, and staff will not be liable for any indirect, incidental, special, or consequential damages arising from your use of our website or its content. Nothing in these Terms limits any liability that cannot be limited under applicable law.

18. Governing Law

These Terms are governed by the laws of the State of Nevada, without regard to conflict of law principles. Any dispute arising from these Terms or your use of our website or services will be brought in the state or federal courts located in Washoe County, Nevada.

19. Changes and Other Terms

  • Changes. We may update these Terms from time to time. The "Last updated" date at the top of this page shows when they last changed. Changes do not affect a service you purchased before the change.
  • Errors. We try to keep information on our website accurate and current, and we may correct errors, including pricing errors, at any time.
  • Severability. If any part of these Terms is found unenforceable, the rest remains in effect.
  • No waiver. If we do not enforce a provision right away, we have not given up the right to enforce it later.
  • Events beyond our control. We are not responsible for delays or failures caused by events outside our reasonable control, such as natural disasters, power or internet outages, or third-party platform failures. We will make reasonable efforts to reschedule affected appointments.
