Do you prescribe hormone therapy?
Absolutely! Hormone therapy is a significant aspect of our practice. We offer various hormone treatments, including estrogen, progesterone, testosterone, and thyroid. However, we do not provide testosterone and estrogen pellets.
Do you accept health insurance?
Our practice values the time and personalized care we give to each patient. Insurance often restricts the time a practitioner can spend with a patient and the type of care provided. Therefore, Revival Medicine does not participate in in-network insurance plans.
We operate on a cash-pay, fee-for-service basis and are not affiliated with any medical insurance companies, making us "out-of-network." Payment is required at the time of service, and we accept most major credit cards. While we do not bill insurance directly, we provide a "superbill" for you to submit to your insurance company. Reimbursement depends on your specific insurance plan and deductible, and we do not guarantee it. We recommend contacting your insurance provider to determine if they will reimburse any of your services.
You can use your HSA or FSA accounts for payment. We do not accept Medicare, Medicaid, or Tricare, and you cannot file claims with these government health plans as Jaclyn Piasta, APRN, has opted out of them.
Do you conduct lab work?
Yes, we partner with Quest Diagnostics and LabCorp.
These centers have locations nationwide and can bill your lab work through your medical insurance provider.
How does it work?
Schedule your appointment above by pressing "Book Now" for the desired service.
Receive an e-mail with required forms and consents to be filled prior to the scheduled appointment.
If your visit requires a video visit, on the day of the appointment, you will receive a text message with the link for your virtual visit. No application download is required. You may attend the telemedicine session via any computer, tablet or telephone that is camera and microphone enabled.
All other services-you will receive a confirmation text one day before and the day of your appointment. We know things happen, if you are unable to make the appointment please cancel 12 hours before.
Your credit card will be charged the total amount agreed upon between you and the provider.
Your provider will send any prescriptions or lab orders following the visit, if indicated.
Can you be my primary care and/or gynecology provider?
Due to the specialized nature of our clinic, we do not offer primary care or general gynecology services. We recommend maintaining relationships with these providers for your preventative healthcare needs outside of Revival Medicine.
Do you accept HSA/FSA?
Yes! You can use HSA and FSA accounts to pay for services at Revival Medicine.
What is the best way to contact Revival Medicine?
